PS4 Slim Won't Play a Particular Game?

I bought NHL 15 pre-owned and put it in my console, it installed fine, but once launching it either comes up with the CE-34878-0 error with the application or states that the disc is dirty and cannot be read. I've already returned one copy of this game and the new pre-owned one looks fine, so I'm doubting that I could possibly have gotten two defective discs. Is this a software issue? Or do I need to get another copy of the game again? The PS4 reads every other game fine.
