I've only played Binding of Isaac co-op on ps4 so I had no idea that on other platforms you could choose what special baby to spawn in as. I did wonder why I would never spawn as a special baby on ps4, but I thought it was just a random chance.
That is until yesterday, I was playing co-op with my friend and by accident I had pulled up the special baby spawn menu. I had no idea how that had just happened and I tried to replicate it. I then went to the online to see if this had happened to anyone else. To my surprise there was little to no information out there. I was only able to find two reddit posts that actually contained the steps on how to do it (and that was out of dozens posts that were left unanswered.)
So all the credit goes to /u/eggroll6 and /u/Overthekitchentop because they're the real MVP's. Ok so here is how to actually do it. When joining in as the co-op player you have to simultaneously click options (the normal button to join) and either left or right on the d-pad. For some reason it's kinda hard to do and it take me like 6 tries before I can do it. But if you've done it right the special baby menu will appear in the top right corner and you can scroll left or right to choose what special baby to spawn in as.
TL;DR To join as a special baby on ps4 co-op, click left or right on the d-pad (also works with analog stick but it's harder) and simultaneously click the options button to join in. Menu should appear and you can choose your special baby.
