PS4 won't read this one disk. Says the disk is either damaged, dirty, or upside-down. Sometimes it recognizes the disk, other times or won't at all. The disk is actually very clean, so I have no idea what to do. I've defragmented the hard drive already, so I don't know what else is going on, especially since this is the only disk this happens to.
