Controller Issue - Square and X Buttons Both Do the Same Thing

I noticed this recently while playing with a friend in NHL. Suddenly, both the X and Square Buttons seem to do the same thing.

What I mean by this is: Hitting X gives the same results as hitting Square and X at the same time. This is true for the reverse. This is just one controller.

I am assuming this is just an issue with the controller (only 6 months old) but it's so strange I'm wondering if there's a possible setting I could change or a fix out there that could help.
