My PS4 keeps deleting games on its own

For the third time now, a game has been deleted from my library without me doing it. I am not accidentally deleting the games, I always close apps by holding down the PS button. Also, no one else has access to this PS4. I have seen many posts about people experiencing this, but no one with a solution. Does anyone have any insight on this? Thanks
