Profile picture and public avatar. just some clarification please.

I need some clarification and a quick question that hopefully some of you awesome people here can help me with. I understand that for your psn account there is a public avatar which in my understanding is the image that is next to your screen name and is what the general public sees(people not on your friends list, strangers in game, like at the end of a round of battlefield or cod ect). These can apparently only be bought on the playstation store, some are free and others are premium and cost money. Then there is your profile picture. These are pictures that appear in the same place as your avatar however they can be your Facebook profile pic(if an account is connect to your psn) or any customer image if uploaded through the ps mobile app. These images however can only be seen by those who are close friends and have sent real name requests to. Is this correct or am i missing something?

Of correct i do have a question. How does one make their profile pic (custom one that only close friends can see) be used as their public avatar? I have searched but have not found a clear answer as most responses confuse the two. Additionally i have read a few responses that say you simply can't there is no Way that you can. I dont think that is try because often at the end of a round of battlefield i see people's avatars that are clearly custom ranging from a Nintendo 64 logo on the interesting side to Nazi images on the less tasteful.
