I was browsing the Bethesda forums earlier and stumbled across this thread where people are discussing the Australian version of the GOTY version of Fallout 4 including only the base game on disc, and a slip of paper with a code to redeem for the DLC.
Another person posted this image of the German version of the game, which also says at the bottom of the back cover of the game that the base game is on the disc, but the DLC is only a code to allow you to download the content.
I tried to dig around to find out of this was the case with North American (or any other) versions of the game and can find nothing saying yes or no. I do notice that every single website that sells the game, or even advertises it, shows only the front artwork, not the back where you would be able to see the disclaimer that you see on the German version about the DLC being a download only.
On eBay, there are people selling the GOTY version of the game with the title ALL DLC ON DISC, but are charging over $150 USD for it, and the stock images they are using show an ESRB rating, so presumably they are offering a NA copy of the game. Again, though, none of these sellers are showing the rear cover art to confirm that the NA version is in any way different from the Australian or German releases of the game.
Just wanted to give a heads up to anyone who has this game pre-ordered, since I know having the DLC on the disc is important to a lot of people (like me, since I have no internet and no way to download the DLC, so pre-ordering the game and ending up with only the base game would suck).
